Autism is a developmental disorder that is multifactorial in nature. Its composition is so complex and diverse that a person who is faced with autism in his family, in order to help his child, begins to study several branches of knowledge at once: psychology, pedagogy, neurobiology, biochemistry, nutrition, and especially meticulous ones, also statistics and mathematics.

10-15 years ago, parents of newly diagnosed children felt the need for one more important knowledge - knowledge foreign language. Most of the information about autism in Russian was limited to information about it as an incurable mental illness, a kind of childhood schizophrenia that requires treatment with a psychiatrist, drug intervention and classes with a speech pathologist.

And only if you spoke English, you could read about various medical studies in the field of autism, linking the disease with problems in the field of neurology, gastroenterology, immunology, biochemistry, and learn about new types of therapy for autism, both in the field of medicine and in the educational and pedagogical space. The richest and most complete source of such information was the website of the Autism Research Institute of America, founded in 1967 by Dr. Bernard Rimland.

On the same website, Russian parents regularly read about annual conferences held by the institute’s specialists for parents of children with autism under the slogan Defeat Autism Now! (“Let’s defeat autism now!”). And then for some of them it became obvious: in order for autism in our country to emerge from a purely psychiatric paradigm, in order for it to become a medical, social and political problem, it is necessary to act, it is necessary not only to translate the materials of American conferences into Russian, but also to transfer the multimodal approach to autism to Russian soil.

At the origins of this process is the parent public non-profit organization “Center for Autism Problems”, headed by two mothers of children with autism, Ekaterina Men and Yana Zolotovitskaya. The International Conference, which has become an annual event, is the most important part of the process of changing the Russian scientific and social landscape of autism.

The American Institute of Autism became a partner of the CPA in organizing the conference, which gladly responded to the offer of cooperation and has been sending its speakers to Moscow for the fourth year in a row.

Another partner was the Immunculus Medical Research Center, headed by the famous Russian scientist A.B. Poletaev. Professor Poletaev and American Autism Institute Director Steven Edelson are co-chairs of the conference organizing committee. Other partners are the press center “Russia Today”, the State Budgetary Educational Institution “School 1465 named after Admiral N.G. Kuznetsov” in Moscow, the Moscow Institute of Psychoanalysis, with the support of the US and Israeli embassies.

The Moscow Conference has a number of advantages that make it unique on our soil. But perhaps the most important are two. Firstly, it is the breadth of topics covered at the conference and the associated diverse competencies of the speakers, and secondly, it is scientific and practical relevance. The conference's presentations and workshops will address topics that are at the forefront of scientific work right now.

Here's an example.

On March 17, 2016, the journal Science published a study by Swedish scientists with a disappointing conclusion: people with autism spectrum disorders live 18 years less than average life expectancy and suffer from various comorbidities, often unable to communicate their suffering to loved ones and doctors.

This is something that parents have been talking about for a long time, but only now their knowledge of the situation has found a voice in the scientific world, thanks to the data of a statistical study. (By the way, statistics can influence attitudes and approaches to autism).

Epilepsy was identified as one of the main causes of early death in people with autism in a Swedish study. The conference program includes a clinical seminar for doctors “Electrogenesis disorders and convulsive conditions in autism: alternative methods management of patients with resistant forms and conditions of unknown origin”, which will take place on April 30.

Several research groups are currently studying how the same risk factors early in brain development can cause abnormalities in neural transmission that ultimately lead to both autism and epilepsy. Although a number of patients suffering from both disorders show a decrease in epileptic activity in response to typical drug interventions, a third of these children have resistant forms of the disease.

The seminar will discuss atypical treatment methods, for example, the ketogenic diet, and therefore it will be led by experts from different fields: American neurologist Kenneth Clayton Sassover from Boston Children's Hospital, nutritionist and dietitian Kelly Bunhill, director of the nutrition clinic at the Center for Child Health and Development in Austin, USA, general practitioner Irina Koshkina, employee of MIS “Immunculus”, Moscow.

Immunology is developing rapidly these days, which, as it turns out, bears fruit at the intersection with almost all other branches of medicine. This is also true for neuroscience, at the intersection of which new approaches to autism are born. Within the framework of the Conference working group under the leadership of Professor A.B. Poletaev will discuss the following topical issues:

  • What is autism in biomedical terms?
  • How to stop its growth?
  • How can the ELI tests and similar technologies developed by Immunculus help with this?

And at the same workshop, Professor of the Department of Pediatrics of RUDN University L.G. Kuzmenko will present a report “Experience in the use of immunoglobulin therapy in the treatment of children with autism spectrum disorders.”

One of the hottest areas of research in recent years is the possible involvement of the digestive system in the origin of autism, in particular, the role of the trillions of bacteria that live in the human intestine. These bacteria and their genes, collectively called the gut microbiome, help break down what the human body can't digest to produce certain nutrients, and influence the immune system in the process.

Of great interest in this regard is the report of gastroenterologist Harold Winter, associate professor of pediatrics at Harvard Medical School at Massachusetts General Children's Hospital, “The Role of the Microbiome in Understanding Complex Diseases Like Autism.”

Dr. Winter is a world-renowned scientist, the author of more than 170 publications. He has received a National Institutes of Health (NIH) grant to study the role of macrophages in gut function, gastrointestinal manifestations of HIV, and inflammatory bowel disease. The report of this outstanding scientist at the Moscow conference is an excellent chance for specialists and parents to get acquainted with the latest scientific achievements in the study of autism at the intersection of three areas: gastroenterology, immunology and neurology.

A unique method of a multimodal approach to solving behavioral and social problems of children and adults with autism will be presented by clinical psychologist from Holland, director of the ReAttach Therapy Institute, Paola Weerkamp-Bartholomeus. Dov Sagi, professor at the Department of Neurobiology and Brain Research at the Weizmann Institute of Science (Israel), will talk about new research data in the field of perception and learning in the autistic brain.

Among the conference speakers who will represent the field of applied behavior analysis this year is the distinguished specialist Stephanie Peterson, Ph.D., professor at Western Michigan University and head of the department of psychology. Stephanie's research interests include behavioral approaches in special education. And Professor of Psychology, Doctor of Science and Behavior Analyst Jane S. Howard (Turlock, USA) will talk about the use of a behavioral approach in organizing the life of the family of a child with autism.

The presentation by our compatriot, Doctor of Medical Sciences, Professor of the Department of Physiology of the Russian National Research Medical University named after N.I. Pirogov, author and translator of a number of textbooks, manuals and monographs by N.N., promises to be interesting. Alipova. Professor Alipov will talk about how a terminological apparatus for describing the foundations and methods of applied behavior analysis is being formed in Russia, which is extremely important due to the fact that the real process of training Russian specialists - behavior analysts and therapists - to work with children and adolescents is currently beginning with developmental features.

A very important issue in working with autistic patients is the choice of treatment in cases of dangerous behavior. In Russia, many adolescents and adults did not have access to behavioral therapy in childhood, and the pharmacotherapy methods used for them were not effective, only temporarily stopping acute manifestations.

Doctor of Science and certified behavior analyst Daniel Shabani (USA) will conduct a two-hour master class “Functional assessment and correction of severe behavioral disorders: overcoming obstacles” as part of the conference. He will introduce specialists to methods for selecting strategies for conducting a functional analysis of dangerous behavior in patients aged 14-20 years in order to draw up an optimal plan for correcting such behavior.

The conference will include a section “With Autism to Independence”, dedicated to the problems of socialization, vocational education and employment of people with autism, in which American, Israeli and Russian specialists will present different models involving people with autism in work activities.

In the section dedicated to the education of children with autism, the most important place will be occupied by reports from Moscow speakers who are simultaneously developing and implementing a model of inclusion, that is, the inclusion of children with developmental disabilities in the learning process in regular classes of a comprehensive school and in groups of regular kindergarten.

The director of Moscow school No. 1465 A.V. Lutsishin, who provided the platform for the implementation of the CPA project, will talk about the transformation of a regular school into an inclusive school, educational psychologist and behavioral therapist N.O. Burlaka and behavior analyst Yu.V. Presnyakov will highlight in their reports the inclusion of students with special needs in preparatory groups kindergarten and secondary school level. The ideologist of the project, president of the Center for Autism Problems, Ekaterina Men, will talk about the concept of the inclusive model and the technology for its implementation.

Within the scope of this article, it is impossible to list all the many topics covered at the conference and talk about all 30 speakers who have high competence and extensive experience in the field of autism.

Read in detail the program of the IV International Scientific and Practical Conference “Autism. Challenges and Solutions” and you can register to participate in its work.

WITH April 19 to April 21, 2018. took place at the press center of MIA "Russia Today" (Moscow) VI International scientific-practical conference"Autism. Challenges and solutions". This is the sixth year the conference has brought together doctors, teachers, psychologists, healthcare and education organizers, and related specialists from all over the world to discuss the most current trends in the field of autism.

The conference program widely covered the relationship between genetics, immunology and gastroenterology and behavioral disorders.

Experts from the field of behavioral sciences and activity-based pedagogy presented the latest evidence-based approaches to teaching individuals with ASD - from early intervention to functional training for adults with autism.

Among those invited to the conference within the framework of programs and methods for improving the psychosomatic state of children with autism, the LATTA-BIO company (Krasnoyarsk) took part with its exposition. A new form of protection against airborne infections aroused great interest among conference participants - .

Along with the bactericidal effect has proven itself well as a means of normalizing the functioning of the central nervous system and as a natural antidepressant. The product aroused interest among specialists, primarily due to clinical and laboratory studies conducted with the participation of children of different age groups on the basis of children's sanatoriums of the Federal Medical and Biological Agency of Russia and the Administration of the President of the Russian Federation.

V Moscow International Conference “Autism: Challenges and Solutions”"was held in Moscow on April 26-28, 2017. It was attended by employees of psychoneurological dispensaries in Arkhangelsk and Kotlas, involved in providing assistance to patients with autism spectrum disorders.

The trip to the conference was made possible thanks to the Russian-Norwegian project “Improving the provision of services to the population with mental and behavioral disorders in remote areas of the Arkhangelsk region” and serves to improve the provision of assistance in this area of work.

The fifth conference, dedicated to a wide range of issues related to ASD, brought together doctors, teachers, psychologists, healthcare and education organizers, and related specialists from all over the world to discuss the most current trends in the field of autism.

The program of this conference highlighted the connection between genetics, immunology and gastroenterology with behavioral disorders.

Organized by the ANO “Center for Autism: Education, Research, Help, Advocacy” together with the American Institute for Autism Research. Its director, Steven Edelson, and the famous Russian scientist Professor A.B. The Poletaevs were co-chairs of the organizing committee.

Relevance

Relevance of the topic: in Moscow alone, according to experts, there are more than 35 thousand children with autism spectrum disorder (ASD), in Russia - about 500 thousand. There are more children with autism than all children with cancer, diabetes, asthma, heart defects, and immunodeficiencies. The reasons are unknown and approaches to treatment/correction are just being formed. The current system of care for patients with ASD in Russia is ineffective and archaic.

Conference participating countries: Russia, USA, Australia, Great Britain, Germany, Holland, France, Italy, Greece, Israel, Turkey, Egypt, Saudi Arabia.

Until recently, autism was considered a disorder of the nervous system and a behavioral disorder and was considered within the framework of psychiatry and neurology. Currently, autism is considered a disorder of the whole organism, where, at its core, it contains a deficit in communication, social interaction, stereotypic behavior and other manifestations that accompany ASD, but in In fact, they are a manifestation of a systemic and complex disease process.

Professor Steven Edelson presented an extensive report, spoke about the history of development and the current state of research on ASD, and emphasized that throughout the world it is necessary to encourage and promote the development of interdisciplinary research on the problem of autism in such areas as epigenetics (genetics and the influence of the environment on it), immunology , Neuroscience, Gastroenterology, Sensory Sensitivity, along with the study of Psychology and Sleep Issues, Child Behavior and Speech and Communication Skills.

Speech by Professor Doctor of Medical Sciences Poletaev, scientific director of the Imunculus Medical Center, echoed the previous speaker and he spoke about new approaches to the early detection of pathological changes in the human body, that an effective tool for preclinical diagnosis of a wide variety of disorders in the body should be a precision (high-precision) analysis of abnormalities in the serum content of many autoantibodies .

Professor A.B. Poletaev believes that in most cases of early detection of incipient, fundamentally reversible pathological processes, the development of events in an undesirable direction can be stopped, and a person who would be doomed to develop a serious disease has a chance to maintain health. It is the study of the spectrum of autoantibodies, reflecting not only the genetic, but also the epigenetic characteristics of an individual, that turns out to be fruitful for predicting his future health and disease. Changes in the content of certain antigens, self or foreign, are reflected by changes in antibodies in the bloodstream, which makes it possible to systematically identify and analyze disorders at the molecule-cell-tissue-organ level.

The professor spoke about the following research conducted. The S100 protein is involved in neurogenesis and morphogenesis in general. The study revealed that an increase in antibodies to the S100 protein causes disturbances in neurogenesis, and this increases the risk of central nervous system pathology in the fetus by 12 times, including autism in the child. For example, excess autoantibodies to the S100 protein are induced by human papillomavirus.

Focusing mainly on the problem of non-genetic multifactorial autism, the professor emphasized that it is impossible to restore a child only by methods of psychological and social rehabilitation, without correcting physiological disorders.

Synergistic methods of correction can be: selection of diet, correction of intestinal microflora (microbiome), removal of excess toxic factors, replenishment of the lack of essential microelements and vitamins, in combination with psychological and pedagogical correction. He provided scientific evidence of the effectiveness of this approach.

Evdokia Anagnostou, a neurologist at the Department of Neurology at the University of Toronto (Canada), presented a study on neuropsychological mapping of impairments in people with autism. A new joint study by American and Canadian scientists has found that many of the children with autism who take antipsychotic medications have never received behavioral therapy. They are immediately prescribed medications that have potentially serious negative side effects, without trying safe treatment options. "This is a wake-up call," she says. We should only use medications when behavioral approaches don't work." Medications are not a good long-term therapy because... their beneficial effects cease when use is stopped, the researchers say.

“The most important task is to teach our children the skills to regulate their own arousal levels, emotions and behavior before we start giving them drugs.

A psychiatrist from the Pitié-Salpêtrière University Hospital, Paris (France), Vincent Gosha and Yoram Boneh, spoke in his report about possible reasons behavioral exacerbations in adolescents with ASD. He presented an interesting fact: in France there is a state psychiatric hospital where autistic people from the most severe states of aggression and self-injury, after a thorough examination, are taken out not with neuroleptics, but with the elimination of somatic problems that provoke such behavior.

Coverage of problems of concomitant somatic diseases in autism spectrum disorders

Doctor of Psychology Katherine Peterson (USA), who combines deep knowledge in gastroenterology and behavioral analysis, spoke about the behavioral approach to eating disorders in children with autism.

Selectivity in food, limited diet, and so on is a very big problem characteristic of autism, which reduces the quality of life of the child and the entire family. She talked about how this problem is being solved in the United States. The child and parents undergo a full-time (8-week?) course with a specialist aimed at correcting eating behavior. Parents then receive support from specialists through telemedicine consultations. Katherine Peterson gave some advice on correcting disorders, and also left her contact information for consultations.

Jill Young talked about natural learning versus the block method. Conclusion from her talk: BOTH styles are effective, DTT can be implemented into the game. At the same time, there are children who themselves prefer a more structured learning style.

As part of the conference “Autism. Challenges and Solutions”, several clinical seminars were held for doctors from leading experts from the USA in the field of autism.

Teachers of the State Budgetary Educational Institution “School No. 1465 named after. Admiral Kuznetsov" of the Moscow Department of Education.

In addition, very interesting reports are presented on topics, in particular:

ABA therapy for autism

The master class was conducted by Jane Howard. Topic: “Working together: cooperation between parents and specialists in ABA therapy for autism.”

Main conclusions:

  • Early diagnosis of autism is important. We can buy time.
  • Family is a very important part of therapy for a child with autism. It is very important when the family works together with a specialist.
  • The principle of constructing work is agreement and consent. If suddenly something goes wrong, then the task of the ABA therapist is to find the cause of the current situation. Find a way out, as this is part of his job.
  • An ABA therapist not only helps the child, he helps the family.
  • Working with families is the ethical responsibility of the ABA therapist. His task is to help the family improve their quality of life so that they can later pass on this experience to other families.
  • Each family has its own individual path of development, path of rehabilitation.
  • Choose your goals wisely.
  • Behavior is the most important part, so parents need to be given a sense of confidence that they can do it, learn a lot of new things and be able to raise their child.
  • The child is always right. The child reflects the environment.
  • It is the job of analysts to involve and motivate parents in the process. Parents of children with autism experience increased anxiety and stress like no other. Therefore, the family needs support. Parents need to remember and understand that they are important and main components of the child’s life. Don't forget to think about yourself and rest.
  • Autism does not happen to a child, autism happens to a family.

A unique two-day master class was presented by leading specialist in adaptive sports and physical education, David Geslak (USA). Listeners were told why the trampoline is called the “king” in the development of gross motor skills, and were shown basic exercises for sensory relief at home, in the classroom and in sports activities.

The Department of Applied Behavioral Analysis of the Moscow Institute of Psychoanalysis also organized other events as part of the non-conference program.
